My child _________________________ has permission to attend the field trip to Seattle Children's Museum on Friday, March 14, 2008 with the preschool class. I understand that transportation will be by private car or school van and that I need to provide a car seat for my child to be left at school that day (labeled with my child's name on it). I also need to bring a sack lunch for my child including a drink.

__________ I am unable to drive this time, and my child will need transportation in the school van or private car, and I will provide a labeled booster seat.
